Step 1. Open and fill out the Application and Informed Consent form below and then save a copy on your computer.
Step 2. Organise required Identification documents which are listed on the Application and Informed Consent form.
Step 3. Once you have completed step one and have organised your ID documents,
simply upload the required documents via the submit section and supply your email address at the bottom of this page.
